
What is an Ontario Health Team?
Ontario Health Teams (OHTs) are groups of healthcare providers and organizations (such as hospitals, primary care providers, home and community care, and mental health services) that work together to deliver coordinated care.
The Timiskaming Ontario Health Team (TOHT) is a collaborative network of health and social service partners dedicated to delivering integrated, patient-centred care in the Timiskaming region.
TOHT brings together hospitals, primary care providers, home and community care services, mental health and addictions supports, long-term care homes, and social service organizations to ensure seamless access to high-quality, coordinated care.
As an Ontario Health Team (OHT), we work to break down silos in the healthcare system, making it easier for individuals and families to navigate their health journey. Our mission is to ensure that people receive the right care, from the right team, in the right setting—when they need it. Through collaboration and person-focused care, we strive to improve health outcomes, reduce barriers, and enhance the patient experience.
Through a local strategic planning process, TOHT identified guiding principles and priorities for the first few years of our work together. We will build on our assets and strengths, empower the voice of community members and challenge each other to remain steadfast in our commitment to people at the heart of every decision and to health and well-being for all
Through this plan, TOHT affirms its commitment to advancing health equity, improving population health, and enhancing access to coordinated, integrated and culturally responsive care.
